Instructions For Emailing Your Reservation Form:

1.  Download the form to your computer

2.  Open the form

3.  Fill out the info and save it to your computer

4.  Email to: caddyman2@comcast.net

5.  Be sure to attach the form to the email

Note: Date, hours, and location will have to be reviewed by John Saville prior to finalization.
